Changes in Medical Utilization Pattern before and after Medical Insurance Program





Abstract
The medical insurance program was started for the business firms with 500 or more employees in July 1977. It has been well known that the medical insurance programs are one of the important factors influencing the magnitude and patterns of medical care utilization.
This study attempted to measure the quantitative changes in medical utilization before and after the
insurance program. Both out-patient and inpatient utilization were analysed in this study.
Annual survey based on structured questionnaire forms were conducted to the employees on daytime
duties during the past 3 years consecutively covering.1 year before and 2 years after the insurance. The total number of study population was (5;502 and-average response rate was 657o. The results were as follows;
1. Average number of OPD visit was 4.9 per person per year, 2.9 before the insurance and 5.5 and 6.9 respectively after the insurance. The increase was 907o and 138% respectively by year after the medical insurance. This trend applied to all groups of employees with differing personal characteris tics.
2. Average number of admission was 3.2 per 100 persons per year, 1.8 before the insurance and 4.2 and 3.8 respectively after the insurance. The increase was 138% and 1117o respectively less pronounced increase during the second year of the insurance.
This trend was more prominent among the male &: manageralclerical workers.
3. When the OPDs are divided into the dispensaries at the working place and the local clinics, the visit to the local clinics has increased dominantly by year after the medical insurance. The change was more prominent among the female and technical-physical workers.
